The ZUPREME-1 trial was a Phase II, randomized, placebo-controlled study evaluating petrelintide as a once-weekly subcutaneous treatment in adults with obesity or overweight with weight-related comorbidities.
Petrelintide produced "double-digit" percentage weight loss versus placebo in ZUPREME-1, with the companies characterizing the tolerability profile as placebo-like. The announcement did not disclose absolute weight-loss figures, confidence intervals, p-values, discontinuation rates, or adverse-event tables, so independent assessment of effect size and statistical robustness is not possible from the data released. Petrelintide is a long-acting amylin analog engineered for once-weekly subcutaneous administration. Amylin, co-secreted with insulin from pancreatic beta cells in response to nutrient ingestion, activates receptor complexes formed by the calcitonin receptor and receptor activity-modifying proteins. That signaling pathway reduces body weight partly by restoring sensitivity to leptin, the satiety hormone, and by inducing earlier feelings of fullness. Wegovy (semaglutide), approved by the FDA in 2021, and Zepbound (tirzepatide), approved in November 2023, have set a high efficacy bar in obesity, with tirzepatide producing mean weight loss of roughly 16 to 22.5% at 72 weeks in the SURMOUNT program. Both agents, however, carry well-documented gastrointestinal adverse event profiles — nausea, vomiting, and diarrhea — that drive dose titration requirements and contribute to discontinuation in a subset of patients. That tolerability gap is the central commercial thesis for petrelintide as a GLP-1 alternative, though the claim rests on Phase II data whose full dataset has not been publicly disclosed.
