Houston-based Cerenome, Inc. (Nasdaq: CNSY) has secured a senior secured financing facility of up to USD 20 million from 3i, LP, structured with royalty-based repayments and designed to fund the commercial scale-up of its cerebrospinal fluid diagnostic platform. An initial tranche was received at closing, with future tranches contingent on milestone achievement.
3i Fund is the sole investor named in the transaction. The company said the facility is structured to be minimally dilutive to stockholders, and that when combined with existing facilities, current cash, and forecast CNSide-related revenue, it expects to extend its cash runway. The financing follows a period of commercial infrastructure build-out: Cerenome selected XiFin Empower RCM for revenue cycle management in August 2026, enabling CNSide billing to begin in Q3 2026, and secured an in-network agreement with Health Care Service Corporation that the company said expanded contracted coverage to approximately 153 million covered lives. The ReSPECT-LM clinical programme is supported by a three-year USD 17.6 million grant from the Cancer Prevention and Research Institute of Texas.
Its CNSide diagnostics platform, commercialized through wholly owned subsidiary CNSide Diagnostics, LLC, is a laboratory-developed cerebrospinal fluid assay that enables quantitative detection and longitudinal monitoring of tumor cells that have metastasized to the central nervous system in patients with carcinomas and melanomas. The company reported 232 CNSide tests performed in the first half of 2026 and received a dedicated AMA PLA billing code effective July 2026. At the 2026 SNO/ASCO CNS Metastases Conference, the company presented data suggesting CNSide could reduce monthly leptomeningeal metastases treatment costs by up to 47%.
Its lead therapeutic, REYOBIQ (rhenium Re186 obisbemeda), is a liposomal nanoparticle radiopharmaceutical encapsulating beta-emitting rhenium-186 for intracavitary CNS delivery. It is being evaluated in three active programmes: ReSPECT-LM for leptomeningeal metastases (NCT05034497), ReSPECT-GBM for recurrent glioblastoma (NCT01906385), and ReSPECT-PBC, a Phase I study in pediatric high-grade glioma and ependymoma (NCT07061626). The underlying science traces to work by researchers including William T. Phillips, Ande Bao, and Andrew J. Brenner at The University of Texas Health Science Center at San Antonio.
