Ocugen, Inc. (Nasdaq: OCGN), a Pennsylvania-based biotech, announced that the US FDA has granted Regenerative Medicine Advanced Therapy (RMAT) designation to OCU410, an AAV5-based subretinal modifier gene therapy targeting retinoid-related orphan receptor alpha (RORA), for the treatment of geographic atrophy (GA) secondary to dry age-related macular degeneration (dAMD). RMAT designation, established under the 21st Century Cures Act, is reserved for regenerative medicine therapies with preliminary clinical evidence of potential benefit in serious conditions. It confers the benefits of both Fast Track and Breakthrough Therapy programs, including rolling Biologics License Application (BLA) review eligibility and the potential for accelerated approval.
The designation was supported by Phase II data from the ArMaDa trial, a randomized, controlled study in 51 patients with GA secondary to dAMD. At the medium dose selected for Phase III, OCU410 produced a statistically significant 31% reduction in GA lesion growth versus control at 12 months (p<0.05), with 55% of treated patients demonstrating at least a 30% lesion size reduction relative to control. An exploratory structural endpoint showed 27% slower ellipsoid zone loss in treated eyes, a biomarker correlated with photoreceptor integrity. No serious adverse events related to OCU410 were reported across Phase I and Phase II, with no cases of endophthalmitis, retinal detachment, vasculitis, choroidal neovascularization, or ischemic optic neuropathy observed. The full 12-month topline data were reported in March 2026.
Unlike the two approved GA therapies — pegcetacoplan (Syfovre) and avacincaptad pegol (Izervay), which target individual complement proteins and require intravitreal injections every one to two months — OCU410 is designed as a one-time subretinal administration that acts across multiple pathways, including oxidative stress, complement regulation, inflammation, and lipid metabolism via RORA. Neither approved complement inhibitor has regulatory authorization outside the US.
