Zealand's petrelintide delivers up to 9.2% weight loss in Phase II obesity and diabetes trial

Denmark-based Zealand Pharma (Nasdaq: ZEAL) reported positive topline results from the Phase II ZUPREME-2 trial of petrelintide, a once-weekly injectable amylin analog being developed with Roche for chronic weight management. The trial met its primary endpoint, with participants receiving petrelintide achieving up to 9.2% mean weight loss at Week 28 versus 2.0% for placebo in adults with overweight or obesity and type 2 diabetes. ​

The 220-patient study demonstrated statistically significant weight loss across all three petrelintide dose groups, ranging from 7.4% to 9.2%. Treatment also produced placebo-adjusted HbA1c reductions of 0.61–0.88 percentage points. Gastrointestinal adverse events were mostly mild, with discontinuation due to these events occurring in 1.9% of petrelintide recipients versus 1.7% on placebo. No unexpected safety signals were reported.

The findings build on positive results from the Phase II ZUPREME-1 trial in people without type 2 diabetes, where petrelintide achieved up to 10.7% mean weight loss at Week 42, compared with 1.7% for placebo. Results from that study were published in The Lancet Diabetes & Endocrinology in September 2026.

Zealand and Roche initiated the registrational Phase III ZUPREME program in September 2026, comprising three trials expected to enroll approximately 7,000 participants across obesity populations with and without type 2 diabetes, and with established cardiovascular disease. The partners are also developing petrelintide in combination with Roche's dual GLP-1/GIP receptor agonist enicepatide (CT-388), with a Phase II combination study planned for H2 2026.
